This sounds like very reasoned and experienced medical advice. But I 
have another question about chest pain: do you docs do automatic 
screening for aortic dissections as well? Probably most of you are 
familiar with the sad story of Jonathan Larson, playwright of Rent. He 
died of an aortic dissection the morning before the show was to have 
opened. He'd been to several hospital ERs, and no doctor discovered the 
life-threatening problem. Why was this? Are only EKGs and cardiac 
enzymes done to r/o cardiac origin of chest pain? 

I have personal reasons for interest in this subject. I'm a survivor of 
an acute dissection (at 4.1 cm), have Marfan, am a cardiac nurse, and 
told the paramedics and ER staff what was going on. (heck, my heart was 
trying to explode) And they still did not take it seriously, at first.

I realize, of course, that acute aortic emergencies (or even those 
incidents where you have some time to plan) are less common than MIs. 
But they are certainly no less catastrophic. 

I was told by a cardiac surgeon friend that all ERs should be equipped 
with TEE capability, done on the spot. I had to wait almost 8 hours from 
the time I was wheeled in the ER door to the time of surgery. (things 
did happen rather more quickly after my CT scan, but then, it's a 
miracle I even survived that long)

Hope that there are some opinions and explanations out there.

I did not mean to suggest this as an entry level examination but as a
possible endpoint of a thorough evaluation.  Obviously a thorough
history will dictate intelligent evaluation.  Resting ECG, Cardiac
enzymes if recent MI cannot be ruled out,  Isoenzymes if the cardiac
enzymes are not definitve,  clinical correlation with exam findings
etc.

Most of the doctors I know are real glad they weren't working in any
of those ERs when he came in.  Figuring out that chest pain in a man
in his 30s without known Marfan's is an aortic dissection would be
really tricky.  Even in an older person with known longstanding 
hypertension it can be a hard diagnosis to make.

It is also, of course, an absolutely imperative diagnosis to make,
which is what makes it such a scary disease for doctors.  There is
no way that everyone who ever comes to an ER with unexplained chest
pain can have an MRA or a cine-angio or a TEE, so doctors have to
use clinical judgement in figuring out who might have a dissection.
Fairly often they fail at this, unfortunately.  Keeping it in mind,
though, is probably the most important part of making the diagnosis.

Permit me a word on the importance of ruling out cardiac disease which
can occur concommitantly with anything else.  A doctor's knowledge is of
considerable importance in the process but so too is the doctor's
thinking.  I believe that the doctor ought to think out loud so the
patient can know what is happening inside his doctor's head.

Perhaps if doctors could recognize remote possibilities when they saw
them, doctors could feel more at ease in keeping their thinking to
themselves.  The problem is that articulation helps the doctor to think,
and frequently thinking, not knowledge, is the critical aspect of the
process.

Remote possibilities?  Remote possibilities are remote until the patient
dies and the doctor is sued for malpractice.  Then the doctor with a
duty to inform the patient is faced with his failure to inform the
patient of the risks the doctor was taking with his patient's life.

My wife had a Charpentier annuloplasty ring done by D. Cooley in the 70's
. I am wondering if she should be on any kind of registery ...for
followup. Her current cardiologist has never mentioned this. She has
regular checkups and has some leakage (mitral)...but noone seems to get
very excited about her findings on echo. Questions:  are these rings done
any more?
                  what is long term survival ?
                  How to get in touch with Dr. Cooley's people...or those
in charge of follow up on his cases?

in Molecular Biology



The project is funded by the TMR (Training and Mobility of Researchers)
and therefore addresses to young scientists who are members of countries
of the European Union. It involves the molecular analysis of
Bradyrhizobium japonicum signal peptidases and their roles in the
establishment and maintenance of symbiotic interactions with soybeans
and other host plants. The main goal of the project will be to analyze
the function of several newly identified genes of B. japonicum and to
study the influence of bacterial mutations on the host plant.

Applicants should have good knowledges in molecular genetics, gene
expression techniques, protein biochemistry and immunology.

Reactive oxygen is a necessary by-product of numerous cellular 
processes, from which the body protects itself with various 
antioxidant strategies. When these protections fail or are 
overwhelmed, the damage to tissue, cells, and genes can be 
extensive. Indications for which antioxidant therapeutics may 
be useful include cardiovascular disorders such as atherosclerosis 
and restenosis; ischemia/reperfusion injury from stroke, heart 
attack, or other sources; ARDS and other respiratory conditions; 
aging and neurodegenerative diseases; certain autoimmune disorders; 
infections; cachexia; and others. Among the therapeutic approaches 
that will be discussed are versions and mimetics of superoxide 
dismutase and other enzymes, stimulation of glutathione production, 
and other radical scavengers, particularly small molecules.

'Protection' in the case of heart attack includes both prevention of
attacks, and reduction of tissue damage when an attack does occur.

Quite a lot of research has been done into drugs and dosage schemes
(i.e., how much of which drug should be taken when), and the field has
advanced considerably over the last few decades. Interesting and
successful new work continues to go on in the field.


Mr. Newman's WWW page (http://www.ridgenet.net/~jamesn/asprin.html)
refers to "a unique and revolutionary flavonoid-based dietary
supplement".  Good nutrition is certainly very important in reducing
the risk of heart attack, but all the peer-reviewed scientific research
that I am aware of suggests that the BEST ways to protect the heart
involve fairly carefully balanced chemical solutions administered at
relatively precise times. "Dietary supplements" might *help*, but if
it were my health, I would not want to count upon them alone.


I would suggest that people with interests in the matter of heart
protection do some investigation, such as reading articles from The
Journal Of Cardiology. Medline should find quite a number of
references, including to some very solid research that we've done here
at the National Research Council Canada, Institute For Biodiagnostics
(http://www.ibd.nrc.ca)

Last December I was hospitalized for a week with Deep Venous Thrombosis 
(DVT).  Recent blood tests showed I have the Factor V gene mutation which 
promotes clotting.  Are there known treatments to help minimize the 
reoccurance of getting another clot?  Any help will be appreciated.

1) Using the WWW to access the BIOSCI/bionet newsgroups.

As of 10 December 1995, all BIOSCI/bionet full newsgroups are
accessible through the World Wide Web (WWW) at URL http://www.bio.net.
One can read and reply publicly or privately to both recent postings
and archived messages through one's Web browser if it is configured
properly to send e-mail.  Each newsgroup is equipped with its own WAIS
index.  The main BIOSCI home page also has access to the BIO-JOURNALS
Table of Contents database WAIS index and the BIOSCI user address
database described in another item further below.


2) What to do about "spams," i.e., junk mail, ads, etc.
-------------------------------------------------------
BIOSCI is a set of parallel USENET newsgroups (the "bionet" groups),
mailing lists, and a hypermail archive at URL http://www.bio.net/.
The same postings are distributed on all media (except for a small
number of mailing-list-only groups at net.bio.net).  Unfortunately it
is becoming a despicable practice on the Internet (by a few people out
to make a fast buck) to do automated mass postings to thousands of
newsgroups and mailing lists.  These attempts to grab free advertising
are refered to as "spams" in the usual, somewhat boneheaded, net
terminology.  USENET is more susceptible to this practice, and many
spams originate on the USENET groups and then are passed on to the
mailing lists.  However, spammers also get lists of mailing addresses
and hit these too, so neither medium is immune.

What should you do personally if you get junk mail?
---------------------------------------------------
Just delete it and move on without reading it further.  Filing a
protest is becoming increasingly useless because spammers are often
disguising the addresses where the messages are sent from.  Unless you
really understand Internet mail systems, your attempt at protest by
sending replies to the message will often end up being sent to the
address of an innocent person that the spammer is victimizing.

What can BIOSCI/bionet do to protect its newsgroups?
----------------------------------------------------
The only solution currently available is to moderate the newsgroup.
If this newsgroup is already moderated, then you are in good shape.
Moderation protects the USENET distribution from about 95% of the
spams that are being sent to date and protects the mailing lists
completely.  Moderation means, however, that someone has to take the
time to review each message before it goes out.  We have set up
software here that simply allows the moderator to forward to an
address at net.bio.net messages that (s)he wishes to have distributed.
This takes no more time than that needed to read the message and pass
it on, say about 1 min. per message.

Most newsgroups currently have a discussion leader who is responsible
for their newsgroup.  The discussions leaders and their e-mail
addresses are listed in the BIOSCI Information Sheet which is
available on the Web at http://www.bio.net/.  If a newsgroup is being
hit with too many junk postings, please contact the discussion leader
for that group and see if there is interest in moderating the group.
Please do not assume that by simply posting a complaint to the
newsgroup itself, anyone on the BIOSCI staff will act on your
complaint.  With close to 100 newsgroups to run, the BIOSCI staff has
to rely on the discussion leaders of each newsgroup to report problems
directly to us at biosci-help@net.bio.net.

We will moderate any of our newsgroups if the discussion leader tells
us that the readership of the group wishes to do so and if a moderator
is willing to do the work.  For most BIOSCI/bionet groups, this
entails only a few minutes of work each day.

Moderating a newsgroup will resolve probably 95% of the junk postings
on the USENET distribution.  Unfortunately there are easy ways for
determined spammers to override the moderation mechanism on USENET,
but we can protect our e-mail subscribers from unwanted postings if
the newsgroup is moderated.  You can also access our newsgroups over
the WWW at URL http://www.bio.net.  While this Web interface will not
stop spammers from trying to post to the groups, this will give you
yet another way, besides using USENET news, to keep the junk out of
your personal mail files.  For those of you with local USENET news
systems, the Web interface will also give you faster access to new
newsgroups and recent postings.


3) Examples of subscribing and unsubscribing to the mailing lists.
------------------------------------------------------------------
PLEASE NOTE: The BIOSCI management does NOT act on
subscription/unsubscription requests that are posted improperly to the
newsgroups and mailing lists.  People who do this only bother everyone
on the lists to no avail.  Please be sure to follow the proper
procedures below.

Gory details are in the BIOSCI Information sheets on the Web at
http://www.bio.net.  Below we give an example utilizing the
METHODS-AND-REAGENTS list at both of our two BIOSCI sites:

A) Determine the "listname" which is the <=8 character mail address
                                         ^^^^^^^^^^^^^
   for the group.  These can be found in the BIOSCI Info. Sheet.  For
   the METHODS-AND-REAGENTS group the mailing address is
   methods@net.bio.net.  The listname is the portion of the address to
   the left of the @ sign, i.e., "methods".  The listname is used with
   the "subscribe" and "unsubscribe" commands illustrated below.

B) Mail all commands in the body of a mail message addressed to
   biosci-server@net.bio.net.  Do NOT send commands to the newsgroup
   posting addresses!  Leave the Subject: line blank, any text on it
   will be ignored.

C) In the body of your message put one or more of the following
   commands with an "end" command on the last line, e.g.,

   subscribe methods
   unsubscribe methods
   end

   Do NOT put your e-mail address or other text on these lines.  The
   server only allows you to cancel your subscription if the address
   on your mail header matches the address on our mailing list.
   Please ask for help at biosci-help@net.bio.net if your address has
   changed, e.g., if you know you are on the list but the server tells
   you that you are not a member.

I am a registered nurse currently working on a cardiology ward at St.
Thomas' Hospital, London.  My work involves the pre and post operative care
of patients undergoing coronary angiography, percutaneous transluminal
coronary angioplasty and coronary rotablation.  Untill recently the policy
on femoral arterial sheath removal was that the Senior House Officer on
call for the ward would remove the sheath and manually press on the wound
untill haemostasis occurred.  Now there is a move to a) have the doctor use
a femoral compression device such as the femostop, or b) have the nurses on
the ward remove the sheaths.  I have attempted to research the efficacy of
femostops without any real success.  I have one research article which
states the use of such devices leaves the patient open to a much higher
risk of haematoma and femoral arterial emboli.  If anyone has any relevant
literature I would be very grateful if they could contact me.

Could anyone direct me to a web site or other reference where I could
find a description (and possibly photos) of heart bypass surgery?  I'm
also seeking a  description, for the family of the patient, of what we
would expect to see when visiting the ICU after the surgery. 

Additionally, are their online support groups for patients and/or
family. AOL and Compuserve sources are ok as well.

I´m a white male 37 years old. Married and have a healthy kid. Work as
CEO.

My problem is that my condition is extremly low. I can´t excercise if
I do little to long walking or slowly running I will be very ill,
shaky, dizzy, palpitations, and hyperventilation feel like dying for
maybe 30 minutes to hours.

The problem dos not occur when I do like lift weight with arms. It
seems that it must be some main body exercise. 

Every time this happen it´s very bad feeling and only way that it not
happen is to think before every move I take.
It seems that the total summary of step (walking) in a day is
connected to the situation when the bad situation occur.

Till my age 20 this hasn´t happen, I was doing many sports when I was
young and didn´t feel this. After 20 it comes more and more often.

Today it´s every time when I do only little short wakling (200
meters). I must take the car everywhere I should go. 

Somebody must had this experience or heard about anybody who has had
the same symptoms.

A. What is the diagnose?
1. Can it be some heartfailure (rest ECG is ok, done 4 times, same
every time) but they said it was a little small jump in every cycle,
but it often is that. I have not done excercise ECG yet or use Holter
ECG but will do soon.
2. Can it be some metabolic failure. It seems that when I have lactic
acid after move. It happen. And also hot temperature (sun) do it
worse. And cold (winter) do it better.
3. Is it "only" psycologic. In my head.
4. Or something else.

B. How can I improve my condition without that this bad situations
occur?

C. Have you heard about any MD who is expert in this area?

it sounds like you need to work with your primary physician to figure
out what is causing your symptoms.  what you describe could be cause by
problems with either the lungs or heart (or mind).  it is too early for
a referral to a specialist.
