Abstract
  BACKGROUND: The RALES study showed that spironolactone,
  added to conventional therapy for chronic heart failure, dramatically reduced
  mortality. We tested the hypothesis that this benefit was partially due to
  improvement in endothelial function and/or to amplified suppression of the
  vascular renin-angiotensin axis. METHODS AND RESULTS: We performed a
  randomized, placebo-controlled, double-blind crossover study on 10 patients
  with NYHA class II to III chronic heart failure on standard diuretic/ACE
  inhibitor therapy, comparing 50 mg/d spironolactone (1
  month) versus placebo. Forearm vasculature endothelial function was assessed
  by bilateral forearm venous occlusion plethysmography using acetylcholine and
  N-monomethyl-L-arginine (L-NMMA), with sodium nitroprusside as a control
  vasodilator. Also, vascular ACE activity was assessed by use of angiotensin
  (Ang) I, with Ang II as a control vasoconstrictor.
  Spironolactone significantly increased the forearm blood
  flow response to acetylcholine (percentage change in forearm blood flow
  [mean+/-SEM], 177+/-29% versus 95+/-20%, spironolactone
  versus placebo; P<0.001), with an associated increase in vasoconstriction due
  to L-NMMA (-35+/-6% versus -18+/-4%; P<0.05). The Ang I response was also
  significantly reduced with spironolactone (P<0.05), with Ang
  II responses unaltered. CONCLUSIONS: Spironolactone improves
  endothelial dysfunction, increases NO bioactivity, and
  inhibits vascular Ang I/Ang II conversion in patients with heart failure,
  providing novel mechanisms for its beneficial effect on cardiovascular
  mortality.

Abstract
  BACKGROUND: Traditional risk factors account for only half of the morbidity
  and mortality from coronary heart disease (CHD). There is substantial
  evidence that oxidative injury plays a major role in the atherosclerotic
  process. Thus, antioxidants may protect against development of
  atherosclerosis. Glutathione, an intracellular tripeptide
  with antioxidant properties, may be protective. METHODS AND RESULTS: This
  case-control study compared total serum
  glutathione (tGSH) in 81 adolescent male
  offspring of parents with premature CHD (ie, before 56 years of age) and 78
  control male offspring of parents without known or suspected CHD. Case
  offspring had significantly lower tGSH than control offspring. In multiple
  logistic regression with parental CHD status as the dependent variable, age
  entered as a covariate, and other CHD risk factors competing to enter the
  model as significant independent predictor variables, LDL cholesterol (odds
  ratio [OR], 2.15 [units=1.5 SD]; 95% CI, 1.21 to 3.82), tGSH (OR, 0.40; 95%
  CI, 0.22 to 0.71), HDL cholesterol (OR, 0.42; 95% CI, 0.22 to 0.78), and
  total serum homocysteine (OR, 2.6; 95% CI, 1.35 to 5.02)
  entered the model as significant predictors of parental CHD status.
  CONCLUSIONS: Low tGSH in adolescent boys is a significant
  independent predictor of parental CHD, in addition to elevated LDL
  cholesterol, low HDL cholesterol, and elevated total serum
  homocysteine concentrations.

  apolipoproteins, and the susceptibility of low density
  lipoprotein (LDL) to oxidation of 45 centenarians (15 men,
  30 women, mean age 101.1 +/- 1.4) living in the Tokyo metropolitan area. The
  average levels of total cholesterol (TC), of LDL-C of high-density
  lipoprotein cholesterol (HDL-C), and of
  apolipoproteins A1 and B were significantly lower in
  centenarians than in healthy middle-aged controls. The frequency of
  hypobeta-lipoproteinemia (apoB < 60 mg/dl) in centenarians
  was almost ten times as high as in controls. The time course of
  copper-mediated LDL, oxidation (assessed by monitoring 234 nm diene
  absorption (lag time)) did not significantly differ between the two groups.
  Analysis of LDL subfractions by non-denaturated gradient-gel electrophoresis
  showed a predominance of large, buoyant LDL particles (pattern A) in 75%, and
  a predominance of small dense LDL particles (pattern B) in 25% of
  centenarians. We also assessed activities of daily living (ADL) and cognitive
  function in the centenarians. Centenarians were divided into two groups
  according to the median ADL score, and were classified into five groups with
  a scale clinical dementia. In subjects with good ADL scores, the mean
  concentration of HDL-3-C was significantly higher than in those with poor ADL
  scores. Average levels of HDL-C were also significantly lower in subjects
  with moderate or severe dementia than in those with normal cognitive
  function. These findings suggest that centenarians have protective phenotypes
  of lipids and lipoproteins that protect
  them from atheroscierosis.

I was wondering if anyone could help me. I am doing a masters research
project into web personalisation services like MyYahoo, MyExcite,
Crayon, and Dow Jones Clips. I am aiming to assess what people think of
these services, whether they help in their work and as regards their
need to find information and in relation to the issue of information
overload.

Hello members,

My mother, three/four days post op (on Monday GMT 4PM) had her aortic
valve replaced at Queen Elizabeth Hospital, Birmingham, by Mr Keogh,
with a carbon fibre St.Jude valve (possibly not a St.Jude but
definatly mechanical).

She seemed to come out of intensive care well, but has spent
a long time (nearly three days) in the high-dependance ward (the
second stage of monitoring here in the UK) while other patients have
spent only a day or two, because her blood pressure is still low.

I am not sure exactly what it is, but the surgeon said it is low
because her heart was so swollen when they operated, and they
hope the heart will return to its normal shape and as a result the
blood pressure return to normal.

Now, they are not sure that the heart will return to normal, and while
her blood pressure doesn't drop as they wean her off her drugs neither
has it risen.

Is this in any way normal, and can anyone spread any light on this
mystery to put our minds at rest.

hello, i am writing to you in reply:  i have a g-mother who had this same
operation in Seattle here in the States.  Same results.  it took some time
but recovery ensued and she is on medication to 'raise' her BP a little.
she has lived eight more years and still going strong.  her meds give her
a little edema but she says 'still around to enjoy family so it is worth
it.'  best of luck.

I`m so pleased you mum has at LAST had her surgery but what a long wait
for you all! Good luck.
Go to: www.google.co.uk and do a search. I`ve just done it by using the
words " alternative warfarin heart patients" A lot of info. came up
which may help. Google seems like a great place for info. searching.
