I think you should not contact anyone, even if her hearing is well within
normal limits. She has not given you permission to forward a copy of the
test results to anyone. To do so without her permission would be a breach
of her confidentiality.
If she should ask you to send results to Social Security and provides you
with written permission (this as much to CYA), then I think your report
should certainly state that her results were inconsistent and what this
implies, etc, as long as you are sure and have ruled out a cookie bite
loss or anything weird.
