Are you trying to determine etiology or appropriate management/rehab? Re: etiology - Is there a family Hx of hearing loss? There is a nonsyndromic genetic deafness known as "Dominant Progressive Hearing Loss" which could fit the general description of this loss, if there is any family Hx.
