Simple program to edit GDB submission forms

I recently wrote, for the benefit of our local users, a program for
editing the GDB electronic submission forms.  I thought it might be
useful to others, thus this note.  The program is written in
Tcl/Tk+TclX, and therefore requires X windows.  (Tcl is available by
anonymous FTP from sprite.berkeley.edu in /tcl.)
