I recently acquired a copy of the program using the address in the back of 
the book.  I have a very recent copy, so maybe the address has changed:

Be sure to specify which format you want (Mac, IBM 5 1/4", or IBM 3 1/2").  
BTW the IBM version is the 9 gene program described in chapter 3, while the 
Mac version is the 16 gene program described in the appendix.
