DATA SUBMISSION FORMS FOR THE GENOME DATA BASE

Data submission to the Genome Data Base (GDB) is carried out on a series
of forms, which can be obtained from a number of sources. The forms currently
in use are:
Cloned Reagent Submission Form.
For submitting Locus (location and assignment mode), Reagent (YAC, cosmid,
phage and plasmid clones) and Contact (name, address, phone etc.) data for
new entries to the database. Any request for a new D-segment should include
a completed cloned reagent submission form (which replaces the old D-segment
submission form).
PCR Submission Form.
For submitting data on primer names and sequences, and on PCR conditions.
Any PCR form defining a new D-segment should be accompanied by a Cloned
Reagent Submission Form.
Polymorphism Submission Form.
For submitting data on alleles, allele frequencies, populations and detection
methods. If the polymorphism is detected with a probe/PCR primers not
already in GDB, the forms for these should also be submitted.
Cloned Gene Submission Form.
For submitting data on cloned genes of known function.
Mutation Submission Form.
For submitting data on mutations, detection methods and populations.
Map Submission Form.
For submitting data on various types of maps (linkage, breakpoint, deletion,
radiation hybrid, FISH etc.).
Contig Submission Form.
For submitting YAC, cosmid, phage and plasmid contigs.

Data submission forms are also available by anonymous FTP from our FTP server
mendel.welch.jhu.edu (128.220.59.42). This carries Postscript versions of the
paper forms, and also electronic submission form templates.
WE STRONGLY URGE THAT DATA SUBMISSIONS BE COMPLETED
ON THE ELECTRONIC VERSION OF THE FORMS.
OBTAINING DATA SUBMISSION FORMS FROM THE FTP SERVER

1. Electronic Form Templates.
The electronic forms are available by anonymous ftp to mendel.welch.jhu.edu
(128.220.59.42) in the directory gdb/submit-data. Connect to this address with
ftp, log in as anonymous, using your email address as the password, then cd to
the directory gdb/submit-data. These forms can be imported into word processors
and text editors for completion.
The files in this directory are:
readme Describes how to complete and submit the forms.
forms.txt Field descriptions for all fields in each form.
gdb_code Coded values required for some fields.
*.frm Electronic forms (templates) for submitting data to GDB.
*.spl Completed sample forms demonstrating proper completion.
Completed forms can be returned by email to data at library.welch.jhu.edu if
no larger than 50kb in size. Submissions can also be returned on Mac or PC
diskette to the address above, or by ftp to mendel.welch.jhu.edu - completed
forms should be placed in the directory /incoming.
PLEASE use these forms ONLY for electronic data submissions. If you submit
data on hard copy, use the paper forms. Please don't print out the electronic
forms, complete them by hand and send them to us.
2. Paper Forms
The paper forms are available in Postscript form from the ftp server. Log in
to mendel.welch.jhu.edu as described above, then change directory to
gdb/submit-data/paper_forms. The eight files here (*.ps) correspond to the
paper submission forms described above (the Contig Submission Form is in two
parts). These can be downloaded and printed on a Postscript printer.

OBTAINING DATA SUBMISSION FORMS BY GOPHER

Both the electronic and paper (Postscript) versions of the data submission
forms will soon be available from Dan Jacobson's gopher hole at
merlot.welch.jhu.edu. Watch for his announcement on bionet.general and
bionet.molbio.gdb.
