1. What method was used before your first Depo-Provera shot to 
ensure that you were not already pregnant.

      a. You told the doctor that you just finished your period.

      b. A laboratory pregnancy test was performed.


2.  How long did your doctor say to wait before relying on the
Depo-Provera. 

3.   Was backup with another contraceptive method 
advised temporarily?

4.  Were you warned about the effects of Depo-Provera on 
a developing fetus? If so, what effects were you told?
