Recipe for linear polyacrylamide

Hi Folks,

Yet another lost reference. Can someone Email me or post a) the 
reference for the above (a single sheet NAR paper) and b) the recipe. I 
can't find it at all in the NAR yearly indexes and it's driving me mad!.
It must be under an unusual key word.
