PCR-primer with T7 Promotor for TK

Dear netters,

I once read in this newsgroup that it is possible to put a T7 
promotor sequence on a PCR primer to in vitro transcribe the 
gene of interest from the PCR product. A few questions regarding 
this issue:

- which sequence do I need exactly to have a functional T7, T3 
or SP6 promotor?

- are unspecific upstream sequences required in order to obtain
 optimal transcription?

- does the transcribed RNA always contain an exact 5'end?
