pRK5 Sequence?

Does anyone know where I can obtain the sequence of the vector pRK5?

It is a mammalian expression vector with a CMV promoter that has been
in th lab for several years, and we only have an old photocopy of the
sequence.  I've tried looking in all the usual databases and have
searched the internet, but can't find it.
