unusual ligation products

I have seen unusual ligation products during cloning. Sometimes one of
the sites has a base or two missing, or the gene ligates into the vector
at sites slightly beyond each restritiction site, or the gene somehow goes
in backwards.
I have seen this with different genes and under different conditions, such
as with and without phosphatse treatment of the vector.
Any ideas on the cause(s) of this?
