dNTP problem

An easy oversight is to forget that when you mix equal volumes of say
.25 of each mM dntp, the concentration of the each in the mix is 0.0625
mM, four fold lower... Could this be the problem?   Otherwise you need
to find the oldest technician in the building and ask him where is the
darkened room with the petagram.  Usually sacrifice of a rat, mouse or
hamster would surfice, but for those problematic pcrs , perhaps a rabbit
or better yet a goat would solve your problem.
