strange cloning results

Hello everybody,

I cloned a SalI / XhoI fragment into a vector cut with SalI and treated
with CIP. 

What I expected was to get 3 different kinds of clones:
- insert in correct orientation
- insert in false orientation 
- religated vector (my CIPing never seems to be complete)
 
What I got (according to restriction analysis) was:
- religated vector (8 out of 24 minipreps)
- vector containing TWO inserts in tandem orientation (16 out of 24
minipreps)

Any idea how this could happen?
