database for chromosomal markers and human diseases

Hi all,
I have been looking for a database that links known chromosomal markers
and human diseases. Has anyone of you come across such a resource? It
needs not be absolutely accurate (linking disease X to marker Y),
stating "correlation between disease X and chromosomal region (band) x
(containing markers ...)" might be sufficient to start from.
