You're most welcome.  According to the Great God Difco, the formulation 
is as follows:

Per Liter of DI H2O, add:

Tryptose, 10 g
Beef Extract, 3 g
NaCl, 5 g
Agar, 15 g
Phenylethanol, 2.5 g

Or you can buy it from Difco.

According to the same source, phenylethanol agar is a selective medium 
used for the isolation of staphylococci and streptococci from specimens 
which may also contain gram negative organisms. The phenylethanol 
inhibits the gram negatives.
