Help needed:Antiobiotics and health questions

1. If a person is diagnosed with pneumonia, how could an antibiotic
be chosen within 24 hours to use against it, if the person has violent
chills and fever of 105? They have chest pains, bloody spit, and
coughing. Choosing an antibiotic in 24 hours is the key. What steps like
enrichment, and selection would need to occur?
2. If the patient from above was from a nursing home, how could one
determine how prevalent the microbe is at the facility, using enrichment
media and a protocol to survey for S. pnuemoniae?
