CAP/CLIA '88 Regulations

When new daily,weekly,monthly reagent/stain qc forms for our 
California clinical microbiology lab were printed, spaces for
lot#/expiration/date of use were eliminated. In CAP or CLIA regulations, are
there specific citations requiring this documentation? It was thought that the
documentation of parallel testing was sufficient recording for lots and dates. 
Your thoughts on this would be most welcome.
