Jonghe's "Phalli...."

Dear mycologists and mycophiles,

I would like to do some work on Renaissance mycological writings and need
help.  Does anyone know where I can find an English (or in some other
modern European language) translation of:

Jonghe, A., "Phalli, ex fungorum genere in Hollandiae....", 1564.

I own a xerox copy, which is quite readable.  I could extend my request to
other works as well, but this will hold me for now.
