Thrush or Angular Chelitis?

I have suffered from a red rash on the corner of my lips for over 5
years now.  The rash sometimes develops painful bumps but never
develops into anything resembling herpetic lesions.  I have been to
numerous Dermatoloigists/Doctors regarding this and no one can tell me
for sure what is causing it!  I have been able to treat it with Mycolog
II which is Nystatin with a anti-inflamatory agent. However, as soon as
I quit applying the Mycolog it comes back!  Also, just recently I have
begun developing very tiny clusters of white spots just under the top
layer of skin on my lips.  From research I've done on the web, it
appears to me that it is either some form of allergic reaction,
recurrent Thrush or Angular Chelitis.  Let me introduce some other
facts.  I am a very healthy individual otherwise.  34 Years old, very
low body fat, heavy muscle tone (into weightlifting/exercise).  I take
multi-vitamins regurally and eat a well balanced diet so it is not
vitamin deficiency.  I have been playing detective for too long now.
Can anyone give me some ideas here?  What could be causing it?  What it
really is? Where can I get more information? etc.?  I need to solve
this mystery!
