Shooting pains when sneezing

What would (severe) shooting pains down the arms and chest when sneezing
indicate? Moreover, I get severe headaches, muscle aches, nausea, and
slurred speech when I get an insufficient amount of sleep (< 7 hours). I am
not sure if the two are related. However, the latter has a tremendous
impact on my career as I have to travel a great deal. It is virtually
impossible to get an adequate amount of sleep when having to catch early
flights. I am afraid that my physician will just tell me to get more sleep.
This may be a workaround. However, most people do not have such severe
reactions to sleep deprivation.
I have a couple of ideas and have scheduled an MRI.this week. One thought
is that I have a Chiari malformation. However, I would like to do some
information gathering before speaking with a neurosurgeon or neurologist.
