NIH Grant Application for WordPerfect or Word

Greetings Nematophilists and Protophilists:
Does any one out there in cyberspace know how or where I can
obtain the current NIH PHS-398 forms on disk suitable for preparing a
self printing grant application. I would appreciate such information for
use with WordPerfect 5.1 (Dos) or Microsoft Word 6.0A (in that order).
Any help or information would be greatly appreciated by not only
me but by our Departmental Secretaries as well. Thanks in advance for
any feedback.
