Problem with O under IRIX 4

< We have a problem with using O with SGI under IRIX 4 here happening quite    
< often.  When I tried to switch from mono to stereo, the display blacks 
< out; the "power saving" yellow light on the display turns itself on; and    
< the display is freezed blank and black.  The machine is still running:    
< you can operate from remote terminals.  But no matter how hard we tried,   
< we cannot recover the display without rebooting.  This does not occur 
< every time I switched from mono to stereo but occurs quite frequently: 
< 2-3 times a day.  I wonder whether this is a generally known problem or it
< is unique to our site and I would appreciate if you could advise us how to
< get around this.
