starting with d4T

My friend is asymtotic 160,200,152 for last 3, 4-month intervals.  His Dr. 
suggests beginning d4T (he is naive to all).  The Dr. suggests d4T has 
greatest effectiveness with least side effects.  My friend could 
choose other AntiVs or enter ProtInhib trials 
or various combo trials but Dr. suggests d4T as a first, conservative, step.

What support is there for a "conservative approach" as proposed?

Also, is there a source for updated pharm and effectiveness info on d4T and 
related substances, esp. dosage levels/body size.

Any advice appreciated.
