question: monoclonal lymphocyte populations

I would like to know about the extent of normal immune responses.

The question is this:

if, in a patient of yours, you find in the periferal blood and in the
bone marrow a monoclonal population of lymphocytes, how many of them are
enought for you to consider the finding abnormal.

Are there completely monoclonal immune responses?
Can the more frequent epitopes in a normal immune response be present in
such numbers as to be mistaken for a neoplasic cell line?

I think not but I´m not sure.
