Smallpox survivors--news media request

Does anyone know why variolation (deliberate infection with smallpox in
cuts in the skin) worked as a vaccine, i.e led to only 1 or 2 vs. 30%
mortality, with immunity?  I believe variola major was used for this.
[This was before Jenner & vaccination.]
