I have a booktitled Therapy with Botulinum Toxin edited by Jankovic and
Hallett (ISBN 0-8247-8824-9, published 1994) which is probably
incredibly expensive to buy but which has a chapter or two on the
treatment of cerebral palsey and spasticity. The chapter on cerebral palsey
is by L.Andrew Koman and colleagues at Bowman Gray school of Medicine,
Wake Forest Uni., Winston-Salem, N. Carolina.

Maybe you should try contacting them if you cannot get a hold of this book.
