mtDNA integration into the nuclear genome

Dear colleagues,
Please suggest references (original research papers, reviews, etc.) 
documenting the integration of mtDNA genes or sequences (e.g. D-loop)
into the nuclear genome, especially of vertebrates.  I am referring to
potential instances where the same or similar sequence (perhaps the
entire 16kb homologous sequence) occurs in both genomes.  This 
obviously would create PCR problems if one did not do a cell fractionation
step or CsCl separation of the two genomes prior to amplification.  Your
help will be appreciated.  Thanks.
